Why does my Audi A2's transmission keep slipping?

I have a 2004 Audi A2 and recently I've noticed that my transmission keeps slipping. It feels like the transmission is struggling to shift gears and sometimes it even jerks when I accelerate. What could be causing this issue and how can I fix it?
